IMPORTANT NOTE:
As our online order form page is still being constructed, you may use our 2008 price sheet, as an order form. While viewing as a PDF or after downloading it, you may 1) print 2) physically fill it out and 3) either:

• Fax your filled out form to: 636.458.4173
• Scan an email your filled out form to orders@patiolivingconcepts.com or
• Mail to us using the mailing address on our contact page.

To save time and paper, you only need to send in the page(s) on which product is being ordered as well as the first page (billing & shipping info) and the last page (price total).
